All of the information you submit to this site is public information. If a public records request per RCW 42.56 https://www.governor.wa.gov/home/privacy-notice is received these records will be released as required.

This form is ONLY for reporting suspected business violations of Governor Jay Inslee’s proclamations and the state Department of Health Orders.

Please read the additional details below before submitting a complaint. Restrictions can be different depending on your location, please verify the business, occasion or gathering is currently not allowed per the current guidelines. Businesses are required to adopt appropriate health and safety measures for staff and customers, and require individuals to wear a face covering when in most public spaces.  You can read more about the Facial Covering requirements here.

DO NOT use this form if you want to report an individual or private group that is not following the Coronavirus requirements. Please contact your local law enforcement agency using their non-emergency number. DO NOT call 911. You may also refer to the law enforcement agency website for directions on how to file a complaint.

Your completed report will be forwarded to the appropriate agency to review the violation for the appropriate response.
